assistance to the community. To apply for a charitable contribution, please
be sure that you meet the requirements:
A contribution to a non-customer organization, officially created for
charitable purposes (i.e., a non-profit organization qualifying for tax-except
status under Section 501 (c) (3) of the Internal Revenue Code or foreign equivalent).
The contribution may not have any strings attached. It may not be tied, in any way, to
past, present, or future prescribing, purchasing, or recommending (including formulary
recommendations) of any drug.
Our online Charitable Contribution Request Form is currently unavailable.
